Work permits are for children ages 14 and 15. The requirements for the permit are:
Proof of Age - Original Birth Certificate, Baptismal Record, or Driver's License
Original Social Security Card
Letter from the Employer (on letterhead) stating the intent to employ the minor along with the job duties, hour of work and time of day the minor will be working. As an alternative, the Employer may use the Work Permit Application.
Letter from the minor's Parent or Guardian, or complete the Work Permit Application
Permit fee is $10.00 to be reimbursed from the Employer.
